To order an AIM Provider Toolkit


  1. Go to: www.healthymichigan.com

  2. Click "ENTER SITE"

  3. Click on IMMUNIZATIONS

  4. Scroll down to: IM106 2011 AIM Provider Toolkit Limit 1

  5. Enter 1 in the box and click ADD

  6. In the right column click Checkout

  7. Complete the form that appears on the left side of the page and hit Submit
